What Drives the Cost
Soil is the main variable. The soil and site evaluation determines what your health district will approve, and it has to be completed before you can even submit a permit application. Heavy clay drains slowly and pushes toward mound systems or engineered designs. Better-draining soils allow conventional systems at the lower end of the range.
System type follows from soil. Conventional leach field systems are cheapest. Mound systems, advanced treatment units and discharging systems cost progressively more, both to install and to maintain.
County requirements vary. Because local health districts may adopt more stringent standards than OAC 3701-29, the same house on the same soil can face different requirements depending which side of a county line it sits on.

Ohio has 88 local health districts and each one sets its own septic fees — there is no state schedule. Six publish figures we could read, and they are reproduced here in full. These are the district’s fees only: the designer, the installer and the excavation are separate, and much larger.

Regional Picture
Northeast Ohio combines heavy clay with a dense population on septic in the outer rings around Cleveland, Akron and Youngstown. Clay pushes many lots toward mounds or engineered systems. Several counties here publish supplemental codes going beyond the state minimum.
Central and western Ohio generally has the most workable soils in the state, and conventional systems are more often permittable. The variable is the fast-growing exurban ring around Columbus, where smaller lots and busier health districts affect both what gets approved and how long it takes.
Southeast and Appalachian Ohio contends with slope, shallow soil over bedrock, and a higher share of older systems. It’s also where off-lot discharging systems are most common, which brings the Ohio EPA permit and annual sampling into the picture.
Permit Fees
Installation and alteration permit fees run roughly $150–$500, varying by county and system type, with alternative systems higher due to additional engineering review.
Under OAC 3701-29, boards of health may also establish additional fees under ORC 3709.09 for the purpose of managing the septic program. Site reviews, re-inspections, and subdivision reviews carry their own charges.

The Cost Discharging Systems Add
If on-site dispersal isn’t possible and your system must discharge off lot, the installation cost is only part of it.
There’s a $200 Ohio EPA application fee for NPDES coverage. There’s an ongoing requirement for a service contract with a registered service provider. And there’s annual diagnostic sampling — TSS, ammonia, dissolved oxygen, E. coli, and chlorine where used — which is the homeowner’s responsibility to arrange, and which not every health district provides.
Those recurring costs don’t appear in an installation quote, and they’re worth understanding before you commit.
Order of Operations
For a discharging system, the sequence runs Ohio EPA first: obtain NPDES coverage, then get the installation permit from the health district. Doing it the other way around creates delay.
For a conventional system, it’s simpler: soil and site evaluation, then permit application, then installation with inspections during construction.
